Please complete the form below to request a transcript or verification of attendance for a current or former student from one of our schools.

NOTE: Record requests are processed within 5 business days. All requested items or requests for additional information will be sent by our Academics Team.

Our organization adheres to the guidelines and restrictions in federal FERPA law, and will not release student records or information to unauthorized individuals. For more information about FERPA, please refer to the US Department of Education's website, which can be accessed here: Family Educational Rights and Privacy Act (FERPA). Only authorized individuals or institutions should submit this form, as attempting to access student records without authorization to do so is a violation of federal law.
Individuals who we do not have listed as approved contacts will need to download and complete a Credential Request Authorization Form . for student information or records to be released. This form can be mailed or dropped off to Ms. Kari O'Connor at 4072 Sunbeam Road, Jacksonville, Florida 32257.
